A 25-year-old male with a severely displaced neck of humerus fracture — at The POSH - Pondy Ortho Speciality Hospital, Pondicherry, early locking plate fixation preserved blood supply and restored complete shoulder mobility.
🦴 View Case DetailsA 25-year-old male presented to The POSH - Pondy Ortho Speciality Hospital, Pondicherry following a traumatic injury to his right shoulder. X-rays revealed a significantly displaced fracture of the neck of the humerus (proximal humerus fracture) — a condition that can compromise blood supply to the humeral head and increase the risk of avascular necrosis (AVN) if not treated promptly.
In young patients, the stakes are especially high. The humeral head receives its blood supply through delicate vessels that run along the fracture line — displacement tears these vessels, and delayed treatment dramatically increases the risk of bone death. Urgent, precise surgical fixation was the only path to a full recovery.
"In a 25-year-old, a displaced proximal humerus fracture is a surgical emergency — hours matter for preserving the humeral head."

"25 years old. Displaced proximal humerus fracture. Locking plate fixation within 24 hours. Zero AVN. Full shoulder function restored — back to daily life."
Post-operative X-rays confirmed excellent fracture reduction and stable fixation. The humeral head was restored to its anatomical position with the locking plate providing rigid, multi-angular stability. Early rehabilitation was initiated under supervised physiotherapy to restore shoulder movement and function.
The patient has recovered exceptionally well, with significant improvement in pain and shoulder mobility. He is currently progressing satisfactorily in follow-up visits and returning to his normal daily activities — a testament to timely surgery and dedicated rehabilitation.
Perfect humeral head alignment confirmed on post-op X-ray
Blood supply preserved — zero avascular necrosis occurrence
Complete shoulder range of motion restored with physiotherapy
Patient returned to normal activities at follow-up visits
This case highlights the critical importance of timely diagnosis and early surgical management of displaced proximal humerus fractures in young patients. The golden window for surgery is narrow — delayed treatment allows blood supply to the humeral head to deteriorate, dramatically increasing AVN risk.
By achieving anatomical reduction and stable fixation using a proximal humerus locking plate and screws, we were able to preserve the shoulder joint, prevent the devastating complication of avascular necrosis, and achieve an excellent functional outcome — allowing this young man to return to full activity.
"Timely surgery + anatomical fixation + early rehab = preserved shoulder, prevented AVN, and a 25-year-old back to full life."
The POSH Hospital, Puducherry remains committed to providing advanced orthopaedic trauma care with a focus on early intervention, optimal recovery, and patient-centered outcomes — giving every patient, regardless of age, the best possible chance at a full return to life.
